Customer Reference - Questions

This is the form used in NC and that we sold for years to customers in all states, in printable form. It has the basic questions and contact info and should be kept on file for each customer. This is in PDF format and prints on a single sheet of paper. Every state is different, so we can't guarantee it will meet the guidelines in your state, but we haven't had a problem with this form in the last 27 years.

When viewing, you can save it to a folder for use later and print them as you need them.

Worried about lamp breakage? We have you covered.

In the event of damage during shipping, we will replace or refund the damaged lamps. Please accept the shipment, note the damage to the carrier, and call us. Fortunately, damage is rare, but we have you covered regardless. Just call us during normal business hours, east coast time, at 1-800-274-1744. You will NOT have to file any paperwork with the carrier or deal with them at all. We handle everything, and can usually fix the issue on your first phone call to us.

We now have a minimum purchase of 14 lamps. If the website isn't allowing you to mix and match properly, just call us at 1-800-274-1744 to place your order.
